Professional Edge Nursing is looking for self-motivated and experienced Care Managers striving for excellence in Care Management.


The Care Managers goal is to educate members on their disease process, teach members how to recognize signs and symptoms, make Doctor appointments, and prevent future hospitalizations. Care Managers use a holistic approach to bring education and awareness to our members and their families. 


Care Managers empower their Members to take a more active interest in their Health Care by, connecting members and families with approved resources, services, and interventions. As well as providing educational materials for each individual health concern. 


EXPECTATIONS:


Care Managers will be expected to provide the perfect experience for the Member. Care Managers will utilize the resources and educational materials provided. Complete the program with the Members per guidelines as well as help guide the Member to the next steps of any other programs that may be available to the Member and family. Care Manager will document all activities within 24 hours of contact with a Member. 


Requirements:


  • RN and Licensed Social Workers must have 1 year post graduate relevant Case Management Experience.
  • LPN must have 3 years relevant Case Management Experience.
  • CCM must have 2 years relevant Case Management Experience
  • Other related disciplines will be reviewed on a case by case basis. Nurse Practitioner, Licensed Professional Counselors, Professional with an MA or BA in Gerontology, Professionals with an MA or BA in Psychology, Sociology, Mental Health Counseling, or Human Services and Counseling will be considered by their relevant experience.
  • 1-year home health experience, critical thinking to address gaps in care, coaching/education, assessments.
  • Geriatric patient work history.
  • Advance computer skills with proficiency with Word, Outlook, Excel, and Electronic Medical Records. 
  • Must be able to travel a minimum of 25-mile radius from home address to members.


ADDITIONAL REQUIREMENTS:

  • Candidates must be well organized, efficient, and able to work independently. 
  • Prompt and Excellent Communication skills
  • Candidates will need to supply a TB test completed in the past 12 months 
  • Must have valid Driver’s License and proof of current auto insurance.
  • High Speed Internet
  • Candidates will have to pass a background and drug screen.

ADDITIONAL INFORMATION:

  • Pay is an all-inclusive* rate of $45 per visit
  • $10 additional awarded for New Member Surveys done within required timeline.
  • Mileage is paid out at a flat rate of $30 for Care Managers that travel 200 miles or more round trip during the bi weekly pay period. Mileage starts from your home to members house and back. This is not per mile reimbursement.
  • 1-week paid vacation after 1 full year from start month/year
  • Pay is Bi-Weekly – You must have Direct Deposit
